About The Position

We are seeking a Home Care Intake Coordinator to serve as the primary point of contact for families enrolling in the Medicaid Home Help Program. This role is ideal for someone with home care, healthcare intake, or Medicaid experience who enjoys guiding families through the enrollment process and ensuring a smooth start to services. You will play a key role in helping families understand eligibility, complete documentation, and move efficiently through onboarding.

Requirements

  • Experience in home care, healthcare, or patient intake required
  • Strong phone communication and customer service skills
  • Experience handling documentation and case coordination
  • Highly organized with strong attention to detail
  • Comfortable working in a fast-paced environment
  • Basic computer skills

Responsibilities
