Home Care Coordinator
About The Position
The Home Care Coordinator supports and implements home care services for participants. The role evaluates home safety through home-visit assessments and coordinates with home-care vendors, family caregivers, durable medical equipment providers, and nursing-home care. The coordinator attends scheduled IDT and coordination meetings and serves as a liaison between the IDT and home-care providers. The position requires leadership and communication skills, along with the ability to identify and understand participants’ medical and social needs when evaluating their home-care needs.
Requirements
- Graduate from an accredited certified nursing assistant program.
- Hold current CNA certification issued by the State of California.
- Have at least one year of experience practicing as a CNA or healthcare worker.
- Have a valid driver's license.
- Hold a valid BLS certification or higher, following American Heart Association or American Red Cross guidelines.
- Possess leadership and communication skills.
Nice To Haves
- Experience working with a frail or elderly population.
- Bilingual English/Spanish, English/Mandarin, or English/Cantonese, depending on location.
Responsibilities
