About The Position

The Home Care Coordinator supports and implements home care services for participants. The role helps evaluate home safety through home visit assessments and coordinates with home caregiver vendors and family caregivers, durable medical equipment providers, and nursing home care. The coordinator attends scheduled IDT and coordination meetings and serves as a liaison between the IDT and home care providers. The position requires leadership and communication skills, along with the ability to identify and understand participants’ medical and social needs to evaluate their home care needs.

Requirements

• Graduation from an accredited certified nursing assistant program.
• Current CNA certification issued by the State of California.
• At least 1 year of experience practicing as a CNA or healthcare worker.
• A driver's license.
• Valid BLS certification or higher, following American Heart Association (AHA) or American Red Cross guidelines.
• Good leadership and communication skills.

Nice To Haves

• Experience working with a frail or elderly population.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location.
