About The Position

LifeStyle Options specializes in customized companion and nursing services and has been providing exceptional home care for over 34 years throughout the Chicagoland area. The Community Care Manager is responsible for coordinating the delivery of services for clients, opening new cases, and ensuring care plans are appropriate for the level of care needed. This role involves supporting the planning, directing, and organizing of service delivery to consistently meet or exceed the company's quality standards, philosophy, expectations, and goals.

Requirements

  • Must have a High School Diploma or equivalent.
  • Home Care experience required.
  • Strong communication skills and interpersonal skills.
  • Ability to work independently.
  • Valid driver's license and proof of insurance is required.
  • Proficient in Microsoft Office Suit.

Nice To Haves

  • Two years health care experience preferred.
  • Certified Nurse’s Aide in good standing with the state is preferred.

Responsibilities

  • Takes initiative to create and implement programs to increase revenue and client population.
  • Responsible for the care coordination of clients.
  • Establishes and maintains relationships with referral sources.
  • Monitor care plans to ensure client needs continue to be met.
  • Performs supervisory visits.
  • Conducts caregiver orientation/training.
  • Addresses client and/or caregiver concerns and complaints in a timely manner.
  • Performs investigation as needed.
  • Ensures timely submission of all required caregiver paperwork.
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department.
  • Follows all HIPAA regulations and requirements.
  • Abides by all regulations, policies, procedures, and standards.
  • Performs other duties as assigned.
