About The Position

The Homecare Manager provides leadership and support to the Home Care teams at our Holland and Muskegon Centers, ensuring the delivery of high-quality ADL and IADL services that promote participant independence, enhance quality of life, and reduce caregiver burden. As a key member of the Interdisciplinary Team, this role oversees the planning, coordination, and integration of home care services, helping participants remain safely in their homes or the least restrictive supportive living environment while ensuring care aligns with the PACE Model and each participant’s individualized Life Plan and care goals.

Requirements

  • Current, unencumbered, and active Registered Nurse (RN) license to practice in the State of Michigan.
  • Minimum of 5 years of supervisory experience, preferably in an interdisciplinary care setting, with at least 5 years of experience serving frail elderly or long-term care populations.
  • Collaborative, team-oriented professional with exceptional communication, interpersonal, organizational, and time management skills.
  • Proficient in Microsoft Office and EMR systems, with strong critical thinking, problem-solving, professionalism, flexibility, and a commitment to compassionate, high-quality care.
  • Current CPR certification through the American Heart Association (AHA) or willingness to obtain certification through LifeCircles.
  • Reliable transportation and a valid Michigan driver’s license.

Responsibilities

  • Develop, lead, and manage systems and processes that guide the operations of the Home Care department at both the Holland and Muskegon Centers.
  • Direct the functions of the home care department in alignment the PACE Model of Care, organizational policies, regulatory requirements, and established budget parameters.
  • Analyze department utilization to ensure efficiency and effectiveness in service delivery.
  • Provide supervision, support, and regular performance-based feedback to team members, fostering professional growth and development.
  • Manage human resource functions for the department, including staffing, performance evaluations, and professional development initiatives.
  • Collaborate with other department managers and team members to promote effective communication and teamwork.
  • Lead project initiatives focused on process improvement and enhanced quality outcomes for participants.
  • In collaboration with the SDIO, develop quality metric outcomes for the home care department.
  • Actively contribute to the annual QAPI (Quality Assurance and Performance Improvement) and Business Work Plans.
  • Ensure integration of the Electronic Health Record (EHR) and supporting database systems into standard workflows for the department.
  • Consult with appropriate leaders regarding high-risk situations and organizational development issues, ensuring proper risk management strategies are in place.
