About The Position

Reporting to the HCC Nurse Manager, the Home and Community Care (HCC)/Public Health Nurse develops, coordinates and implements in-home nursing services for HCC clients residing in KO communities, and works with a team of Community Personal Support Workers (PSWs), Healthcare Professionals, KO team and Community Leadership. This individual also directly provides in-home, client-centered nursing services to clients. Additionally, as a Public Health Nurse, the nurse will provide health care services to support the needs of school age children, youth, and community. The PH nurse will help to support broader community health and wellbeing that includes immunizations, rapid testing, case tracing and case management. PH nurses will be utilizing a holistic approach within the communities. PHNs will work in First Nations communities to identify and address social determinants of health to support individual, group and community resiliency. The HCC/PH Nurse fulfills all responsibilities in accordance with the standards of practice and guidelines of the College of Nurses of Ontario.

Requirements

  • A Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
  • Minimum two years of experience providing nursing care in-home or in a community setting and public health.
  • Excellent verbal and written communication skills in English.
  • Basic computer literacy skills and willing to learn new cEMR system.
  • Ability to prepare reports and submit reports.
  • Willingness and ability to travel to the KO communities.
  • Awareness of working with Unregulated Health Workers; transfer/delegation of skills, processes, and requirements.
  • Knowledge on culture and tradition of remote First Nation communities.

Responsibilities

  • Develops, coordinates, and implements public health education/in-home nursing services under the direction of the HCC Nurse Manager, and in conjunction with Community Leadership and/or KO Community Health Director.
  • Defines program priorities and develops and implements related strategies.
  • Ensures that the services provided meet the needs of clients through ongoing program monitoring, and consultation with clients, community members and leaders.
  • Planning and delivering a Public Health Program taking into consideration Indigenous culture and traditional values which influence individual, family, and community.
  • Conducts program evaluations and identifies any areas of concern and makes related recommendations to the HCC Nurse Manager.
  • Develops and maintains a system for data collection and record keeping.
  • Prepares statistical reports to the HCC Nurse Manager on a quarterly basis and/or as needed.
  • Contributes to the development and review of the service delivery plan.
  • Works with a team of Personal Support Workers (PSWs), and other healthcare professionals in the program.
  • Conducts home visits and assessment of client needs; creates a care plan for the PSWs to follow in the community setting.
  • Provides ongoing administrative and clinical supervision, guidance and support to Community PSWs and ensures optimal level of service delivery is being maintained.
  • Support communication and engagement between schools, families, community health & education worker within the community.
  • Creates training plans and contributes to ensuring clinical competencies are met.
  • All requests for supplies and equipment to be brought to the attention of the HCC RN Manager, who will include each request on an HCC master inventory sheet. The HCC RN Manager or other, will then work together with KO staff in making shipment arrangements to the communities.
  • Provides in-home, client-centered nursing services for KO HCC clients and continues to provide nursing care within Public Health related matters.
  • Assesses client needs and develops goal-based, client-centered client care plans; care plans for the Community PSW as well as Nursing and Case Management care plans.
  • In-home nursing services may include but are not limited to the following: acute care nursing (e.g. complex dressings, injections), wound management, medication monitoring, managing chronic diseases, advanced foot care, palliative care, therapeutic diet support, mental health coordination and teaching clients to care for themselves.
  • Networks and maintains relationships with medical professionals in the community, the Chief and Council, and other stakeholders.
  • Consults and collaborates with the KO HCC Nurse, KO community Nurse in charge, regarding clients, staff, and other matters.
  • Consults with and/or makes referrals to healthcare professionals and KO Health programs (e.g. IRS, Mental Health, Crisis, NP’s) as needed.
  • Attends meetings related to Public Health/Home & Community Care or as requested by HCC Nurse Manager & KO Health Director and provides verbal reports when requested.
  • Establishes linkages with other organizations and/or individuals for the program.
  • Attends workshops, training, and other professional development initiatives to ensure nursing skills remain current.
  • Assists HCC Nurse Manager with written quarterly and year-end reports for KO, Health Canada, NAN, MOLTC and other stakeholders (i.e. LHIN, Public Health).
  • Performs other duties requested by the HCC Nurse Manager or KO Health Director.
