About The Position

UPMC Family Hospice is hiring a Home Care Aide to provide designated care in patients’ homes under the direction of the primary service provider. The role supports identified patient goals and functions as an extension of skilled nursing and/or rehabilitation services in the home. The position serves the Irwin, Murrysville, and Greensburg area and follows a daylight schedule with weekend and holiday rotation. Mileage reimbursement and benefits are offered.

Requirements

• High school diploma or GED.
• At least 75 hours of Nurse Aide training; applicants without prior Nurse Aide training may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
• CPR certification meeting AHA standards, including didactic and skills-demonstration components, within 30 days of hire.
• Automotive insurance.
• Driver’s license.
• Act 33 clearance.
• Act 34 clearance.
• OAPSA clearance.

Nice To Haves

• One year of experience.
• Certified Nurse Aide.
• Nurse Aide Registry in good standing in the state where the facility is located or a state covered by a licensure compact agreement with that state.

Responsibilities

• Communicate appropriately and promptly with the interdisciplinary home care team, reporting changes in a patient’s condition to appropriate professional staff.
• Attend compliance training and follow organizational standards of conduct, policies, and procedures.
• Provide safe, direct personal care to patients in the home under the direct supervision of an appropriate professional.
• Deliver services in keeping with the organization’s service management philosophy toward patients, visitors, staff, peers, physicians, and other departments.
• Identify, develop, and participate in process-improvement opportunities to enhance home health agency service quality.
• Maintain and complete documentation accurately and promptly in the patient’s home and in accordance with agency and regulatory standards.
• Participate in required agency meetings.
• Maintain annual competency evaluation scores as determined by regulatory requirements.
• Facilitate change.
