About The Position

The Field Supervisor / Personal Care Services role is responsible for providing supervision and leadership to Home Care Aides. This position involves evaluating the quality and effectiveness of services provided to clients, conducting home visits to ensure the plan of care is followed, and coaching, counseling, mentoring, and evaluating the performance of Home Care Aides. The role also includes investigating allegations of fraud and theft, and documenting and following up on incident reports.

Requirements

• High School Diploma or GED required
• 2 years of Home care, caregiving or CNA Experience
• Strong communication, organizational, and problem-solving skills.
• Must have valid driver’s license, reliable transportation, and auto insurance.

Nice To Haves

• Bilingual skills a plus, but not required.

Responsibilities

• Provides supervision and leadership to Home Care Aides
• Evaluate the quality, effectiveness and of services that are needed for each of the clients served by HCA
• Conduct unannounced quarterly home visits with clients that have Paid Family Caregivers to ensure the plan of care is being followed and HCA's are present during the hours scheduled.
• Coach, counsel, mentor and evaluate the performance of HCA's
• Conduct home visits as necessary, to provide hands on training and assistance
• Initiate and/or participate in client staffing discussion with Care Coordinator
• Investigate allegations of fraud and theft reported by HCA
• Documentation and follow up on incident reports
