About The Position

Siouxland Community Health Center has an opening for an HIV Program Case Manager. This is a full-time position, Monday through Friday, 8 AM to 5 PM. The successful candidate will provide case-management services, including intake, assessment, care plan development, and follow-up for RW Part B and C program patients. This role involves care coordination, linking case management with access to care, and conducting home visits. The position also requires advocating for patients regarding specialty referrals, housing, appointments, ADAP, prescription regimens, transportation, social services, pharmaceutical companies, and support groups. Additionally, the case manager will perform HIV and Hepatitis C testing, both in-house and in outreach, and conduct social networking to identify new diagnoses. Other duties and projects may be assigned.

Requirements

Bachelor Degree in social work, psychology, or related field preferred
Current licensure in the State of Iowa as appropriate to degree.
Strong communication skills
Strong facilitation skills
Ability to work in a fast-paced environment
Strong written skills
Highly accurate work
Strong computer and data entry skills
Travel to rural areas within the service area to provide case management services for Part B patients.

Bilingual in Spanish, strongly preferred.

Responsibilities
