About The Position

Siouxland Community Health Center has an opening for an HIV Program Case Manager. This is a full-time position, Monday through Friday, 8 AM to 5 PM. The successful candidate will provide case management services, including intake, assessment, care plan development, and follow-up to RW Part B and C program patients. This role also involves care coordination, linking patients with access to care, and performing HIV and Hepatitis C testing. Travel to rural areas within the service area is required to provide case management services for Part B patients. The position also includes other duties and projects as assigned.

Requirements

  • Bachelor Degree in social work, psychology, or related field preferred
  • Current licensure in the State of Iowa as appropriate to degree.

  • Travel to rural areas within the service area to provide case management services for Part B patients.
  • Strong computer and data entry skills.

Responsibilities

  • Provide case-management including intake, assessment, care plan development, and follow up to RW Part B and C program patients.
  • Provide care coordination, linking case-management with access to care to include home visits.
  • Advocate for patients regarding specialty referrals, housing and housing assessments, appointments, ADAP, prescription regimens, transportation, financial, social services, pharmaceutical companies and support groups.
  • Perform HIV and Hepatitis C testing, i.e., “Free and Confidential Testing,” both in-house and in outreach, including conduction social networking to identify new diagnosis.
  • Other duties and projects as assigned.
