Requirements

  • Associates Degree or Bachelor Degree in Health Information Management
  • 3 - 5 years experience working in HIM dept

Responsibilities

  • Assists HIM Director/Operations Manager in directing and evaluating compliance to privacy, information security, and confidentiality of health information standards
  • Responsible for the daily supervision, assignment of tasks related to analysis. Monitoring the integrity of the content and completion of the medical records to ensure the timely analysis of records deficiencies and Chart Correction/Merges.
  • Responsible to monitor Transcriptions and pending Operative Reports/Notes and Queries and encourage completion to support coding and hospital reimbursement.
  • Prepares monthly MEC Report for monthly presentation; prepares repors for MMAC of "Administratively Closed Records" of physicians who are deceased, no longer on staff or retired.
  • Run HIM End of the Month Statistical Report; Generate and distribute physician outstanding deficiency reports in EPIC/ Report manager; generate and distribute mid-level outstanding deficiency reports in EPIC/ report manager.
  • Complete audits using Epic and OneContent report to maintain the delinquency rate within regulatory compliance standards.
  • Serves as primary HIM liaison with the HIM-Epic team to review, correct, and improve record deficiency workflows and reports, for build and maintenance of work queues in EPIC/One Content;
  • Understands workflow and demonstrates the ability to delegate to team leaders and coordinate team member within analysis, transcription, ROI functions and occasionally coding.
  • Work with Operations Manager to coduct Departmental meeting and document meeting agenda/minutes for Director.
  • Communicates effectively with staff through written directives, staff meetings, training and individual feedback.
  • Responds to staff questions, concerns, and suggestions
  • Represents the director in meetings as assigned.
  • Attend Forms committee meetings, Legal Medical Record committee meetings, Patient Safety Commettee meetings and assist in the creation of presentation dealing with HIM topics during House Staff Orientation and Residents Orientation.
  • Conduct performance annual and probationary evaluations within the time frame specified by Human Resources.
  • Directly supervises staff, including hiring, evaluating and disciplining.
  • Perform quality assessments of staff functions to assure compliance with processing of patient's health records; measure staff productivity and quality; ability to assess situations and adjust workflow to ensure all areas are covered.
  • Ability to assess situations and adjust systems workflow to ensure all areas are covered; resolve minor technical issues associated with EPIC/One Content work queues; utilize basic IT troubleshooting processes to identify the root cause of HIM workflow problems and departmental equipment; utilize IT help desk for problem resolution where necessary to maximize efficiency and effectiveness.
  • Demonstrates knowledge to identify potential areas of risk surrounding One Content/ Epic systems: knowledgeable of quality assessments required for One Content/EPIC; knowledgeable of EPIC HIM deficiency tracking and completion; EPIC Hospital Chart Completion process; HIM Chart tracking protocol; EPIC Chart correction process; monitor Epic Merges.
  • Assist Operations Manager in monitor physicians incomplete record activity, including notifying physicians of incomplete status and suspension status; maintain on-going review of mid-levels delinquent record count in the master listing; complete General and Operative suspensions for physicians and residents.
  • Oversee HIM chart review process for internal and external customers.
  • Follow security and privacy policies and procedures to use the networks, including intranet and internet; follow confidentiality and security measures to protect the EHR
  • Prepares weekly Operative Reports/Queries, ED Providers' Note, Hospitalists and Psy Reports.
  • Responsible for the weekly Email notification to Administration, providers and Office Managers to assist in the timeliness of all chart completion.
  • Manage electronic timekeeping functions for employees; monitors time, approves request for time off, overtime and exceptions to work schedules.
  • Is able to review and responsible for monitoring monthly/quarterly audits to assess regulatory compliance with completeness of data and information, timeliness, accuracy, and authentication requirements on records previously audited by the HIM Team Leader.
  • Assists the Director/Operations Manager with the development/revision of policies and procedures related to medical record data and documentation integrity and data management, including standards, usage, operations, documentation, training, interfaces, security, and integrity.
  • Work with the Compliance department to ensure the uploading of all department policies are on the global site.
  • Monitors Chase transcription performance including resolution of issues preventing the interface of documents into the electronic medical record (Epic).
  • Monitors Analyst data integritry, merges duplicate medical records process to ensure the integrity of the patient record and the status of duplicate creation and validate all potential patient matches.
  • Identify corrective action to decrease the number of duplicate medical record numbers created.
