About The Position

Atlantic Health System is seeking a HIM Inpatient Coder for a remote, part-time position of approximately 22.5 hours per week. This role involves accurately coding inpatient records, capturing all diagnoses, POA indicators, and procedures, and validating key UHDDS elements. The coder will abstract pertinent information from patient records to accurately reflect the patient's encounter. A key aspect of the role is reviewing CDI concurrent review notes and collaborating with the CDI Team to ensure optimal code capture and reimbursement in compliance with CMS policies and procedures and Official Coding Guidelines. The position also requires implementing the AHS physician query process when documentation is inadequate, ambiguous, or unclear for coding purposes. The coder will be responsible for completing coding of all discharges/encounters, meeting a minimum productivity requirement of 10/day with a 95% accuracy rating, and handling assignments including Inpatient acute care, Inpatient Psych, Inpatient Hospice, and Newborn. Monitoring the Coding Priority WQ throughout the day to clear cases daily, ensuring no cases remain on WQs for more than 3 days, and staying updated on coding guidelines and reimbursement reporting requirements are also essential. The coder will work with their assigned Lead to review coding questions, engage in coder feedback sessions, attend all coding meetings, and bring identified concerns to leadership. Adherence to the Standards of Ethical Coding as set forth by the American Health Information Management Association is required. Other duties as assigned/necessary will also be performed.

Requirements

Accurate coding of inpatient records, capturing all diagnoses, POA indicators, and procedures.
Validation of key UHDDS elements.
Abstraction of pertinent information from patient records.
Review of CDI concurrent review notes.
Collaboration with the CDI Team for optimal code capture and reimbursement.
Compliance with CMS policies and procedures and Official Coding Guidelines.
Implementation of AHS physician query process for unclear documentation.
Completion of coding for all discharges/encounters.
Meeting minimum productivity requirement of 10/day.
Maintaining a 95% accuracy rating.
Coding for Inpatient acute care, Inpatient Psych, Inpatient Hospice, and Newborn.
Monitoring Coding Priority WQ to clear cases daily.
Ensuring cases do not remain on WQs for more than 3 days.
Staying updated on coding guidelines and reimbursement reporting requirements.
Working with assigned Lead to review coding questions and engage in feedback sessions.
Attending all coding meetings.
Bringing identified concerns to leadership.
Abiding by the Standards of Ethical Coding set forth by AHIMA.

Responsibilities

Codes inpatient records capturing all diagnosis, POA indicators, and procedures and validates key UHDDS elements as well as abstracts pertinent information from patient records, to accurately reflect the patient’s encounter.
Reviews CDI concurrent review notes and works with the CDI Team to ensure optimal code capture on all cases. Ensures optimal reimbursement in compliance with CMS policies and procedures and Official Coding Guidelines
Implements AHS physician query process when code assignments are not straightforward or documentation in the medical record is inadequate, ambiguous or unclear for coding purposes
Completes coding of all discharges/encounters ensuring the minimum productivity requirement of 10/day is met as well as a 95% accuracy rating. Assignments include Inpatient acute care, Inpatient Psych, Inpatient Hospice, and Newborn
Monitors the Coding Priority WQ throughout the day as to clear cases each day.
Ensures cases never remain on any WQs for >3 days.
Keeps abreast of coding guidelines and reimbursement reporting requirements
Works with their assigned Lead to review coding questions and engage in coder feedback sessions; attends all coding meetings.
Brings identified concerns to leadership
Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association
Performs other duties as assigned/necessary
