The High Intensity Care Coordinator is responsible for assessing recipient needs and strengths, identifying individualized goals, and planning for appropriate services. This role involves monitoring recipients to ensure they are placed in the least restrictive environment and tracking their progress. The coordinator will advocate for recipients to secure community resources and collaborate effectively with treatment team members and outside sources to achieve goals. Ensuring services are meaningful, respectful, and promote self-determination is key. The position requires demonstrating youth/young adult specific competencies, participating in the development of crisis intervention and support plans, and assisting participants in achieving independence, employment, educational goals, and community functioning. Transportation assistance, psycho-educational services, and co-leading multi-family groups are also part of the role. The coordinator will also perform other duties as assigned within the Nova Mobile/Nova Washington First Episode Psychosis Program team.
