NYCP is a large hunger relief organization that provides food assistance, case management, nutrition education, and hygiene services. They utilize a holistic approach to poverty reduction, aiming to establish long-term stability for their clients. The Social Care Network Resource and Referral Case Manager will work with various database referral platforms to connect households with appropriate services within NYCP, addressing social determinants of health. This role involves monitoring referral systems, assessing needs, connecting clients to services, providing follow-up, and maintaining reporting tools. Additionally, the position includes conducting benefits screenings and assisting with public benefits applications such as SNAP, TANF, Safety Net, and Rental arrears.
