About The Position

Sage Mountain Senior Living is seeking a compassionate and experienced Health & Wellness Director to lead their clinical team and ensure exceptional care for every resident. This role is for a licensed nurse who enjoys mentoring others, building meaningful relationships, and making a lasting impact. The Health & Wellness Director will lead and support the wellness team, foster a positive, resident-centered culture, and oversee resident assessments, care plans, and clinical documentation. They will partner with residents, families, physicians, and community leaders to ensure outstanding care, ensure compliance with state regulations and Agemark policies, and supervise medication management and nursing services. The role also involves hiring, coaching, training, and developing wellness team members, responding to resident concerns and emergencies, identifying opportunities to improve resident health, safety, and quality of life, and serving as Manager on Duty as needed.

Requirements

  • Current RN or LPN license in California.
  • Minimum one year of experience working with seniors.
  • Two or more years of leadership or management experience.
  • Strong clinical assessment and medication management knowledge.
  • Excellent communication, leadership, and organizational skills.
  • Ability to build relationships with residents, families, and team members.
  • Comfortable using computer systems and electronic documentation.

  • Flexibility to work occasional evenings, weekends, and on-call responsibilities as needed.

Responsibilities
