About The Position

Sage Mountain Senior Living, an Agemark Senior Living community, is seeking a compassionate and experienced Health & Wellness Director to lead their clinical team and ensure exceptional care for every resident. This role is ideal for a licensed nurse who enjoys mentoring others, building meaningful relationships, and making a lasting impact.

Requirements

Current RN or LPN license in California.
Minimum one year of experience working with seniors.
Two or more years of leadership or management experience.
Strong clinical assessment and medication management knowledge.
Excellent communication, leadership, and organizational skills.
Ability to build relationships with residents, families, and team members.
Comfortable using computer systems and electronic documentation.
Flexibility to work occasional evenings, weekends, and on-call responsibilities as needed.

Responsibilities

Lead and support the wellness team while fostering a positive, resident-centered culture.
Oversee resident assessments, care plans, and clinical documentation.
Partner with residents, families, physicians, and community leaders to ensure outstanding care.
Ensure compliance with state regulations and Agemark policies.
Supervise medication management and nursing services.
Hire, coach, train, and develop wellness team members.
Respond to resident concerns and emergencies with professionalism and compassion.
Identify opportunities to improve resident health, safety, and quality of life.
Serve as Manager on Duty as needed.
