Job Summary The Health Transformation Clinical Pharmacist provides clinical pharmacy support focused on ambulatory care, specialty services, medication access, and HealthConnect One (HCO) patient care initiatives. This position collaborates closely with physicians, nurses, care coordinators, and interdisciplinary healthcare teams to optimize medication therapy management, improve patient access to care, and support innovative healthcare transformation initiatives. The role supports current and future specialty-focused programs including oncology, cardiology, rheumatology, dermatology, behavioral health, transitional care, and other ambulatory service lines. This position also serves as a patient-facing pharmacist supporting HealthConnect One pharmacy services and medication access programs.
