About The Position

Provides health education in schools and communities. Serves as a lifestyle coach and coordinates general health promotion activities, including education events for the Public Health Nursing department. The goal is to improve the well-being of patients and the population served. This position is located within the Tohono O'odham Nation Health Care and its facilities. The Director of Public Health Nursing serves as the supervisor for this position. This position may require temporarily relocation to other TONHC Facilities: Sells Hospital, Santa Rosa Health Center, San Simon Health Center, Casa Grande, and San Xavier Health Center.

Requirements

Bachelor's Degree in Health Education, Public Health or closely related field and two years' work experience as a health educator, diabetes educator, or an equivalent combination of training, education, and work experience demonstrating the ability to perform the duties of this position.
Current certification in Cardio Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED).
Must obtain a certificate of completion for Diabetes Prevention Program Lifestyle Coach Training within six months of hire.
Must obtain Food Handler's Card within six months of hire.
Upon recommendation for hire, a criminal background and a National FBI fingerprint check are required to determine suitability for employment, including a 39-month driving record.
May require possessing and maintaining a valid driver license (no DUIs or major traffic citations within the last three years).
If required, must meet the Tohono O'odham Nation tribal employer's insurance requirements to receive a driver's permit to operate program vehicles.
Knowledge of the Tohono O'odham culture, customs, and traditions.
Knowledge of applicable federal, state, tribal laws, regulations, and requirements related to health education.
Knowledge of education principles to plan, implement, and evaluate a comprehensive school health education program K-12 and behavior modification programs concerning health, nutrition, and physical fitness.
Knowledge of tribal, Bureau of Indian Education, and public school systems.
Knowledge in the use and operation of audiovisual equipment and computers.
Knowledge of various health topics such as nutrition, diabetes, physical activity, alcohol/drug abuse, tobacco, safety, injury prevention, and violence prevention.
Knowledge of motivational interviewing techniques to elicit behavior change.
Knowledge of safe and sanitary food preparation and storage.
Knowledge of Basic Life Support to maintain a current CPR certification.
Knowledge of Health Insurance Portability and Accountability Act of 1996, Public Law 104-191 (HIPAA).
Skill in analyzing problems, projecting consequences, identifying solutions, and implementing recommendations.
Skill in operating various word-processing, spreadsheets, and database software programs.
Skill in providing superior customer service to external and internal customers.
Ability to create and present effective speeches and presentations.
Ability to maintain privileged, confidential information.
Ability to work independently and meet strict timelines.
Ability to communicate efficiently and effectively both verbally and in writing.
Ability to establish and maintain positive and effective working relationships with others.
Ability to work extended hours, weekends and various work schedules.
Ability to travel within the interior/exterior boundaries of the Tohono O'odham Nation.
Regularly required to talk or hear.
Frequently required to sit; walk; use hands to finger, handle, or feel, reach with hands and arms; stoop, kneel, crouch, crawl, climb or balance.
Lifting and carrying of educational materials and audiovisual equipment (not to exceed 30 pounds) are common.
Physical exertion during physical activity, prolonged periods of standing, and frequent travel throughout the Tohono O'odham Nation.
Specific vision abilities required by this job include close vision, distance vision, peripheral vision, and ability to adjust focus.

Nice To Haves

Certification as an aerobics instructor preferred or other certification demonstrates the ability to lead physical activity safely and effectively.
Based on the department's needs, incumbents may be required to demonstrate fluency in both the Tohono O'odham language and English as employment conditions.

Responsibilities

Coordinates with a health educator to plan school health education, for grades Pre K through 12 students.
Develops/selects curricula and lesson plans used in teaching and promoting a healthy lifestyle to meet the needs of local school-age children and community members.
Provides technical assistance to schools, community, and tribal programs to improve health and physical fitness, such as providing health education curricula, resources, assessing body mass index, and conducting a walking program.
Provides health promotion/disease prevention education to community, staff, schools, and youth to increase public awareness of the benefits of healthy eating and physical fitness to prevent/manage diabetes and other chronic diseases.
Serves as a lifestyle coach to deliver a chronic disease prevention curriculum to adults and motivates participants to make lifestyle changes to prevent diabetes and other chronic health conditions.
Communicates and collaborates with counterparts in health care, communities, schools, tribal, state, and federal programs to improve the population's health status by conducting health activities and participating in collaborations.
Prepares and distributes health education material such as lesson plans, brochures, and culturally relevant and age-appropriate flyers.
Utilizes the Electronic Health Record (EHR) system to verify patient visits and other health education encounters.
Maintain accurate documentation; writes narrative and statistical reports on activities.
Collaborates with and makes referrals to other departments or agencies as needed.
Maintains professional knowledge by attending seminars, educational workshops, classes, webinars, and maintains certifications.
Conducts community outreach and health related presentations.
Understands non-clinical roles and assists where needed.
Participates with local community groups in the planning of activities.
Operates GSA other department vehicles; sometimes over hazardous roads or in hazardous weather conditions.
Maintains compliance with all mandatory training requirements.
Integrates concepts of Tohono O'odham cultural values, beliefs, and practices as appropriate.
Assists with inventory, purchasing, ordering, storing supplies, and ordering program supplies or promotional items.
Performs other job-related duties as assigned and contributes to a team effort.
