Requirements

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Incumbent may have access to records containing Social Security numbers in the performance of their job duties.

Nice To Haves

  • This position in our TB Program requires the ability to serve a large percentage of Spanish speaking population. Therefore, the incumbent in this position prefer bilingual; fluent in English, Spanish, or Creole
  • Current Certification as a CNA, MA or Phlebotomy.

Responsibilities

  • Works detecting, controlling, and preventing communicable disease county wide.
  • The incumbent in this position will be required to travel throughout Polk/Hardee County to provide client services for TB patients. Must have and maintain an operable automobile, a valid Florida Driver’s license, and valid automobile insurance. Will be expected to occasionally transport patients in State car or incumbent’s personal car. This position requires the incumbent to work independently to carry out the day-to-day duties.
  • Performs screening and follow up on Refugee Clients, including notification, Labs, Data Entry.
  • Conducts initial and follow up interviews, contact investigations, performs targeted testing, provides patient education.
  • Makes home visits, delivers and observes the ingestion of prepackaged, pre-labeled medications for TB Cases, suspects, and LTBI patients, and high-risk contacts.
  • Monitors for side effects and drug toxicity, reports patient’s status to Nurse Case Manager, documents findings in patient record. Promotes adherence and follows up on delinquent patients.
  • Collects data regarding patients and contacts for the purpose of contact investigation, program statistics, and data entry.
  • Communication with other Healthcare facilities regarding TB/LTBI Cases for proper treatment and follow-up.
  • Assists in clinic open clinic visit in HMS collecting vitals, lab specimens, chaperoning, translating and preparing client for provider s including Case, INH, Refugee, and Walk in. Assists with coordination of scheduling for medical management, and X-ray appointments for clients. Instructs patients regarding services available, location, and time of appointments, assists patients with transportation needs.
  • Assists with delivery and pick-up of medical records and x-rays.
  • Performs phlebotomy, collects sputum specimens, and collects other laboratory specimens as needed for patient/program management. Performs Data Entry of patient information for labs. Performs HIV pre and posttest counseling.
  • Assists with clerical duties, registers clients, for refugee clients utilizes most current Refugee Health Program Guidelines, uploading/scanning documents, tracking Refugee services on a spreadsheet, completes administrative duties such as completion of time sheets, mileage logs, employee activity records, and data entry into HMS.
  • Prepare, process, package, and ship UN3373 shipments via FedEx meeting IATA and ICAO requirements.
  • Stock and place orders for lab supplies and Fed Ex
  • Upload TB medical CDs into Infinite
  • Performs other related duties as required, including performance of interpreter duties consistent with skill level as needed. Provides clerical support when needed.
