About The Position

The Health Resource Specialist will assist the Health & Mental Health Manager in the planning, coordination, and implementation of the health services program. This role involves conducting health screenings, monitoring health status, maintaining community resource lists, coordinating health-related visits, and distributing health information. The specialist will also assist with health education needs, attend meetings, support dental and nutrition programs, and contribute to reporting and provider relationship management. Additionally, the role may involve assisting with child and family enrollment and providing translation services.

Requirements

HS Diploma or equivalent
One year experience in a Social Service Agency
Current First Aid and CPR cards
Desire to work with low-income children and their families
Ability to establish relationship of trust and respect with staff, families, and children
Knowledgeable in eligibility requirements
Must possess a valid driver's license and personal automobile or provide a plan that will allow fulfillment of position requirements.
Current enrollment in the Child Care Division’s Central Background Registry
Current physical examination, drug screen, and TB screen documentation prior to hire.
A DMV Record check will be conducted prior to hire.
Communicate effectively with staff, families, children, and the public using the telephone and in face-to-face, one-to-one, and in group settings.
Fluent in English both verbally and written.

Responsibilities

Conduct vision and hearing screens with children and re-screenings as needed, notifying relevant staff of referrals for specialists.
Conduct immunization reviews and monitor children's immunization status.
Assist with coordination of First Aid kits and updating emergency postings.
Maintain a list of community health resources to connect families with Medical and Dental Homes and follow-up care.
Coordinate and/or facilitate health-related home visits with parents.
Assist in providing current health screening and treatment information to data entry personnel.
Assist with ongoing assessment of health education needs and distribute health information to children, staff, and parents.
Attend health-related meetings as requested by the Health & Mental Health Manager.
Assist the Dental Hygienist with coordination of Fluoride Varnish Application and Screening.
Provide monthly health reports to the Health & Mental Health Manager.
Assist the Health & Mental Health Manager in maintaining effective relationships with community health care providers.
Assist with the recruitment, selection, and enrollment of children and families.
Provide translation and interpretation of program materials during parent meetings, trainings, conferences, and home visits.
Ensure each child receives a nutrition assessment, including growth assessment, hemoglobin check, diet assessment, and nutrition education.
Ensure each child receives a Lead Screen and necessary follow-up.
Facilitate the flow of information from WIC nutrition assessments to Early Head Start and Head Start centers using the Nutrition Assessment Record.
Provide a monthly report to the Nutrition USDA Manager on the number of complete nutrition assessments by site and children on WIC.
Participate in staff meetings, conferences, training sessions, and workshops.
Maintain congenial and respectful relations with staff, children, families, and the community.
Keep current and accurate records and file reports on time.
Maintain confidentiality of staff and family information.
