Requirements

  • Bachelor’s degree in Social Work, Business Administration, Public Health, Nursing or related field
  • Advanced and Progressive Experience Developing and implementing multi-institutional programs, partnerships and supervising staff and contractors
  • Serving vulnerable or marginalized populations/communities
  • Program/Project management and problem solving, data analytics, quality improvement and financial optimization
  • Driving success effectively within a matrix leadership structure and shared leadership environment

Nice To Haves

  • Master’s or Doctoral degree in Social Work, Business Administration, Public Health, Nursing or related field preferred
  • Experience in Working With Populations including but not limited to the uninsured, communities of color, non-English speaking populations, those with access barriers including disability, age, poverty, geographic disparity, literacy and other factors is preferred.
  • Federal grant administration for multi-site projects and complex programs.

Responsibilities

  • Providing advice, guidance, and leadership to RHM and Region leaders in developing strategies and in the achievement of performance goals.
  • Enable Collaboration across and within System Services area, RHM and Regions to ensure consistency and integration of strategy and operations.
  • Actively guides, trains, mentors and monitors colleagues while contributing to the development and execution of RHM, Regional, Service Area and System Office based strategic plans.
  • Providing advice, guidance, and leadership to System Office, Service Area, and RHM.
  • Leading standardization/system-ness and optimization of policy, process, methodology, establishing a national community of practice.
  • Oversee Vendor/Contract Labor Management including centralizing strategy and optimizing spend.
  • Collaborates in system-wide strategy development and deployment of functional area priorities and initiatives.
  • Responsible for supporting regional efforts to comply with functional area priorities.
  • Accountable for the selection, evaluation and overall success of the functional leadership teams.
  • Organization-wide focal point for establishing functional strategies and governance over financials and staffing.
  • Accountable for communication between System Services functional area, Service Area, Regional, and RHM leaders.
  • Responsible for measuring and reporting KPIs/metrics and value delivery.
  • Providing advice, guidance and leadership for the colleague life cycle.
  • Maintains a working knowledge of applicable federal, state and local laws / regulations, Trinity Health Integrity and Compliance Program, and Code of Conduct, as well as other policies, procedures and guidelines in order to ensure adherence in a manner that reflects honest, ethical and professional behavior and safe practices.
  • Enables enterprise strategy to address internal or external business and regulatory issues; provides functional expertise and ensures fulfillment of performance and service standards.
  • Identifies, defines and solves complex problems that impact the management and direction of the business; Collaborates with Service Area, Regions and Health Ministries to ensure consistency and integration of strategy and operations.
  • Provides decision support, operations and / or optimization leadership focus.
  • Has responsibility for making decisions and managing risk that impact day-to-day regional, service area, enterprise or health ministry functional operations, with potential for a short- to mid-term impact on the enterprise as a whole.
  • Oversees a direct staff that includes leaders with direct reports or oversees large / complex programs without direct reports.
  • Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership behaviors, practices and decisions.
  • Convenes and maintains a multi-sector team of Trinity Health system office, local Regional Health Ministries (RHMs) and external partners to a) establish a network of in-person, distance learning and virtual CDC-recognized program delivery organizations and standardize procedures for participant engagement and support; b) develop and test a variety of models for NDPP and other social care program insurance coverage; and c) share learnings among all stakeholders.
  • Employs the principles of health equity to focus collaborative efforts on engagement and social needs support for populations experiencing disparities in health care access and health outcomes.
  • Supervises CDC grant-funded system staff and directs contractors/national partners either directly or matrixed with other Trinity Health departmental leaders.
  • Develops and manages budget of approximately $2.5 million per year, including funds distribution to RHMs and external partners.
  • Collaborates with Finance, RHMs, partners and system team to ensure all required financial documentation and reporting are completed accurately and on time.
  • Serves as the primary Trinity Health representative to the CDC Project Officer, maintaining routine communication, participating in scheduled meetings and submitting documentation and reporting accurately and on time.
  • Ensures that NDPP delivery, growth and reimbursement and all grant-related activities are well aligned and integrated with Trinity Health’s Social Care structure and strategy.
  • Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
