About The Position

The Health Promotion and Outreach Specialist conducts targeted member outreach related to health promotion, wellness, chronic condition management, maternal and child health, substance use, tobacco prevention, and other population health initiatives using various communication modalities. The specialist provides education on available benefits to address unmet member healthcare needs and refers members to Care Coordination, Care Management, and primary care as needed. This role is also responsible for advancing a System of Care approach by collaborating on member-focused education and training, enhancing cross-functional collaboration, promoting access to essential services, and analyzing trends for continuous quality improvement. This position is full-time remote and requires the selected candidate to reside in North Carolina. Some travel for onsite meetings may be required.

Requirements

  • Bachelor’s degree in public health, health promotion, or related human services from an appropriately accredited institution and three (3) years of relevant experience working with individuals with behavioral or physical health needs.
  • OR
  • Associates in public health, health promotion, or related human services from an appropriately accredited institution with at least five (5) years of experience working with individuals with behavioral or physical health needs.
  • Relevant experience must include: Cultural responsiveness, Health literacy awareness, Trauma-informed communication.
  • Knowledge of Medicaid basic, enhanced MHSUD, and waiver benefits plans.
  • Knowledge of and skilled in the use of Motivational Interviewing techniques.
  • Demonstrated exceptional interpersonal skills, highly effective written/verbal communication ability, and the propensity to make prompt independent decisions based upon relevant facts.
  • Ability to translate technical health information into plain language.
  • Strong problem solving and conflict resolution skills.
  • Data driven mindset—proficient with Excel.
  • Proficiency in Microsoft Office products (such as Word, Excel, Outlook, etc.).
  • Experience with Electronic Medical Records.
  • Ability to work independently while adhering to Alliance policies and deadlines.
  • Flexibility and adaptability.

Nice To Haves

  • Two (2) or more years of experience in community health promotion, outreach, or population health within a Medicaid or state funded setting.
  • Experience with Medicaid population in North Carolina preferred.
  • Certified Community Health Worker (CCHW) credential preferred.
  • Motivational Interviewing.
  • Training or experience in whole person care.
  • Experience with electronic medical records.

Responsibilities

  • Conduct proactive and targeted outreach using phone, text, portal messages, email, and mail.
  • Review and manage member worklists for health promotion campaigns.
  • Update member contact information and communication preferences in Jiva.
  • Explain benefits and services in a clear, compassionate, and culturally responsive manner.
  • Educate members on preventive care, EPSDT services, and chronic condition supports.
  • Identify barriers to care and act as a liaison with internal teams to resolve barriers to preventive care.
  • Provide information, education, and consultation to organizations serving individuals with special health needs or high-risk populations to enhance access to public healthcare services and support the implementation of System of Care approaches.
  • Plan, support, and implement community health promotion and outreach initiatives that address priority population health needs and align with organizational goals.
  • Collaborate with community-based organizations, providers, schools, and other stakeholders to increase awareness of available services, strengthen partnerships, and improve access to care.
  • Assist in the development and dissemination of culturally responsive health education materials, outreach strategies, and community-facing resources.
  • Deploy interventions based on identified member trends and emerging health needs in a community setting.
  • Facilitate linkage to Care Coordination, Care Management, Member and Recipient Services, and community resources.
  • Support members in accessing primary care and other essential services.
  • Facilitate referrals and assist in the early identification of individuals with special health care needs.
  • Document all outreach and engagement accurately and in accordance with departmental standards.
  • Maintain compliance with organizational policies, procedures, workflows, and the Service Engagement Model.
  • Document in the CM Platform System (Jiva); other systems as identified.
  • Comply and assist with the implementation of organizational Processes, Workflows and fidelity of Service Engagement Model.
  • Travel between Alliance offices, attending meetings on behalf of Alliance, participating in Alliance sponsored events, etc. may be required.
