In this role, you will lead the development, implementation, and evaluation of Personal Care Services and Home and Community-Based Waiver programs, ensuring individuals receive high-quality, person-centered support. You will participate in service planning by reviewing the Consumer Assessment Tool, support plans, and any requested amendments, contributing directly to thoughtful and individualized care decisions. Additionally, in this role you will also monitor and evaluate service providers to ensure their practices comply with CMS guidelines and support consistent, high-quality program delivery. Requirements
- A Bachelor's degree from an accredited college in biological, health or behavioral science; health practice; education; public, healthcare, or business administration; or a closely related field; AND/OR Experience performing health program planning, development, coordination, evaluation, or implementation; providing technical health care assistance and consultation; conducting health care utilization or quality assurance examinations; and/or delivering health care.

- Writing: Recognizes or uses correct English grammar, punctuation, and spelling; communicates information (e.g., facts, ideas, or messages) in a succinct and organized manner; produces written information, which may include technical material and information that is appropriate for the intended audience.
- Community Dimensions of Practice: Distinguishes the roles and responsibilities of governmental and non-governmental organizations in providing programs and services to improve the health of a community.
- Public Health: Applies knowledge of the concepts, principles, theories, methods, and tools associated with protecting and improving the health of people and their communities, including promoting healthy lifestyles, researching disease and injury prevention, and detecting, preventing, and responding to infectious diseases.
