About The Position

The Health Information Management Specialist II will organize and secure the confidential management and handling of all paper and/or electronic member records to ensure compliance with HIPAA Privacy and Security requirements. Adheres to all CCANY and Departmental policies and procedures to ensure that proper consents/record releases are in place when processing requests for access to member records including internal and external document requests.

Requirements

  • Bachelor’s degree in Health Information Management or related field and 1 year of experience working in document management is required, or;
  • Associate’s degree in Health Information Management or related field and 2 years’ experience working in document management is required, or;
  • High School diploma/equivalency and 4 years’ experience working in document management.
  • Proficiency with the use of excel and other software products used for tracking the flow of information.
  • Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
  • Excellent interpersonal, communication organization and demonstrated ability to manage multiple tasks simultaneously.
  • Ability to work autonomously.
  • Demonstrate professionalism, respect, and ability to work in a team environment.

Nice To Haves

  • Prior experience working with Mfiles or Core Solutions CX360 platforms preferred.

Responsibilities

  • Complies with requirements of HIPAA, NYS OPWDD, NYS DOH and/or other accreditation entities with oversight authority for Member Records management in a CCO/HH environment.
  • Assists in the monitoring and release of confidential information in accordance with CCANY’s policies and procedures, federal regulations, and NYS statutes on privileged information requests from members, authorized representatives, insurance companies, attorneys.
  • Assists in complying with subpoenas.
  • Confers with CCANY’s HIPAA & Compliance Officer and/or General Counsel for record releases to auditing investigators including Office of Inspector General, MFCU, DOH and other governmental oversight agencies (and on any requests for information where there is question/doubt about releasing/disclosing confidential member information).
  • Works collaboratively with Care Management, Quality Improvement, and Compliance to provide input on Member Record policies and procedures.
  • Assists in the monitoring and implementation of policies and procedures for Member Records.
  • Provides day to day document management as it relates to Member Records including compiling, indexing, and filing of member records, as well as document retention and destruction protocols.
  • Works collaboratively with Care Management, Compliance, Quality Improvement and Learning and Development to assist in the implementation of training on Member Record topics as needed.
  • Works collaboratively with Care Connection team to create Electronic Health Records for new members.
  • Assists in planning, organizing, and executing the activities of the department.
  • Ensures records are filed in a timely manner and using prescribed naming conventions.
  • Assists the Care Management, Quality Improvement and Compliance staff in the gathering of information related to audits and/or document reviews.
  • Organized and able to meet deadlines.
  • Performs other duties, as assigned.
