The Program Supervisor IV (Health Information Management or HIM Director) performs highly complex (senior-level) administrative and supervisory work, managing the overall goals and policies for the Medical Records department, including both the Electronic Medical Record (EMR) and other required documents related to patient care and treatment. Oversees ongoing records reviews and work-flow management. Assures compliance with requirements from the accrediting and certification agencies, Texas Administrative Code (TAC), and other relevant standards. Oversees the release of information in response to record requests, including requests from the court. Serves as the hospital privacy officer, assuring that the entire hospital remains in compliance with HIPAA standards. Serves as internal consultant on health information management issues (e.g., release of information, confidentiality, information security, information storage and retrieval, and record retention, etc.), as well as integrity and authentication of health record documentation and emergency preparedness for the record system and department. Supervises the work of others. Works under limited supervision, with moderate latitude for the use of initiative and independent judgment. Performs other duties as assigned. Compiles, reviews, verifies, and reports the daily census information to designated hospital staff and hospital leadership. Develops an organized chart tracking and storage system including a process for ensuring retention and de-activation based on retention schedule compliance. Oversees and monitors timely and thorough medical record reviews and takes action when discrepancies are found related to the integrity and timeliness of the record and information system. Oversees patient information flow and ensures compliance with release of information requests within statutory requirements. Promotes and maintains standards for security conscious awareness and behavior related to protecting Protected Health Information (PHI). Serves as HIPAA Privacy Officer for the hospital. Oversees the preparation, reconciliation, and closing of all medical records. Ensures discharged client records are reconciled/finalized within the specified time frames. Verifies the collection, copying, and distribution of medical record requests by mail, fax, secure server, or email. Supervises and manages assigned staff. Responsibilities include, but are not limited to, reviewing applications, interviewing candidates, selecting and hiring potential employees, scheduling and assigning workload, establishing performance measures, evaluating work performance, providing feedback and guidance, recommending staff development and training, recommending personnel and disciplinary actions, scheduling and approving leave. Participates and serves on the Medical Records Advisory Committee (MRAC), as well as other hospital performance improvement activities as requested. Provides technical and functional assistance to staff as well as other general services to various internal (clinical and administrative), and external (e.g., DSHS, HHSC, and regulatory agencies) customers as needed, including assisting with special projects as they occur. Serves as internal consultant on health information management issues (i.e., release of information, confidentiality, information security, information storage and retrieval, and record retention), and standardization of processes associated with medical records. Ensures the integrity and authentication of health record documentation and emergency preparedness for the record system and department. Assures that all inpatients are on a legal commitment. Serves as the record authority in court as legally required and in certification/accreditation surveys. Oversees the flow of documentation necessary for all client admissions, registration, continued stay, and discharge medical records. Coordinates with hospital staff for the client’s continuity of care and oversees the discharge packets (preparation and distribution). Oversees the release of information in response to record requests, including requests form the court. Performs other duties as assigned.
