About The Position

This is a senior level professional work whereby incumbents perform a variety of assignments in the areas of (1) community liaison/development/advocacy or (2) social services work among client populations (e.g. youth, aged, community action, housing development, health, etc.). Incumbents may perform the more complex community development assignments subject to extensive public contact. Work is performed under the general supervision of a Community Developer IV or other designated official. Work performance is reviewed in terms of production, timeliness, and the successful completion of assignments.

Requirements

  • Bachelor's Degree from an accredited college or university in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field.
  • Two (2) years of responsible community development supervisory work.
  • Must possess and maintain a valid Class C driver's license.
  • An equivalent combination of education, experience and training which provides the required knowledge, skills and abilities may be considered.

Responsibilities

  • Plan, develop and implement program to help community clients prevent and manage [their] chronic disease(s) with measurable results.
  • May lead a group of subordinate personnel engaged in collecting and analyzing pertinent data relative to local, state, or federal community development related programs.
  • Coordinate/prepare special projects and reports as required.
  • Conduct presentations and coordinates exhibits to support outreach and education activities.
  • Develop a variety of evidence-based outreach, promotion, and educational resources, using print, social media and web tools.
  • Collect and analyzs data to monitor impact of program outreach of the decrease of specific chronic diseases in the county.
  • Perform complex senior level assignments in programmatic areas.
  • Assist in the production of educational products that increase awareness of the prevention and management of chronic diseases.
  • Educate residents about behaviors that promote wellness through trainings and workshops.
  • Promote evidence-based chronic disease prevention and management programs and provide health educational resources during community outreach events.
  • Assist with the development and coordination of all program activities.
  • Provide technical assistance to PreventionLink DPP Umbrella Hub Subsidiaries to include resource referrals, online support, and curriculum planning.
  • Participate in partnership meetings and other forums.
  • Perform other tasks and duties which may not be specifically noted but are included in the classification level typically associated with the class of work to meet department guidelines.
