About The Position

Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center (FQHC) committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US health care system requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations, and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population. The Health Educator is responsible for planning, developing, monitoring, and evaluating health education programs for the county. The Health Educator will assess healthcare needs, develop health education programs, and perform targeted healthcare education interventions. The duties also include coordinating with healthcare specialists and interest groups to meet the health education needs of patients.

Requirements

  • Experience with Managed Care/Hospital work experience required
  • Knowledge of the Care Process and ability to apply all steps.
  • Comprehensive knowledge, application of medical therapy, human behavior, and techniques for effecting behavior change.
  • Comprehensive knowledge and application of support services.
  • Critical thinking to integrate facts, informed opinions, active listening, and observations.
  • Decision making, problem solving, and collaboration.
  • Excellent counseling skills and ability to relate to a multi-ethnic community and varied learning levels.
  • Strong interpersonal skills to establish productive working relationships with a multidisciplinary team and support services.
  • Ability to communicate effectively in both written and verbal form to patients, the public, medical staff, and physicians.
  • Ability to organize tasks and function independently.
  • Ability to counsel and motivate others, as individuals and as a group.
  • General knowledge of nutrient analysis, word processing, spreadsheets, and professional presentation software.
  • Maintains patient, employee, and Foundation confidentiality at all times, discussing patient or employee business only with appropriate parties who have a bona fide need to know; and communicating only the minimum amount of information necessary with respect to protected health information (PHI) as defined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Responsibilities

  • Provide health and wellness education.
  • Plan and implement programs and treatments specifically designed to address the patient's needs.
  • Evaluate, design, present, recommend, and disseminate culturally appropriate health education information and materials.
  • Perform intakes on new and re-enrolling adults in various programs.
  • Facilitate classes according to the organization's guidelines.
  • Maintain patient logs of all who are scheduled for procedures.
  • Consult with physicians and staff regarding related health education services.
  • Present information effectively to diverse audiences.
  • Other duties as assigned
