Incumbents are responsible for a program area which includes new program development and/or revision and implementation of current programs. Positions may act as support staff or lead person for other health related programs. Positions receive indirect supervision within a framework of departmental policies and procedures. Performs a full range of complex tasks within a framework of established procedures. Within the Children & Families Commission, the position supports the development, coordination, implementation, and evaluation of early childhood health and family-support initiatives, including health and developmental screening, care coordination and referral systems, community outreach, and emerging Medi-Cal/CalAIM service-delivery infrastructure for children prenatal through age five and their families. The ideal candidate will have experience working within early childhood, public health, healthcare, or family-serving systems and a strong understanding of the needs of children prenatal through age five and their families. The successful candidate will bring experience with health, developmental, and/or behavioral screening and referral; family engagement and community outreach; care coordination; program development and implementation; data collection and outcome monitoring; and cross-sector collaboration. Experience with Medi-Cal, CalAIM, Community Health Worker services, Enhanced Care Management, Community Supports, closed-loop referral systems, or other healthcare reimbursement and service-delivery models is highly desirable. The ideal candidate will be a collaborative, adaptable systems thinker who can translate program requirements into practical workflows, build strong relationships with families and community partners, and help develop sustainable infrastructure that improves access to services and outcomes for young children and families.
