About The Position

FreedomCare is a healthcare company dedicated to revolutionizing the home care industry. We support patients by ensuring they have the power to choose a caregiver who will care for them in the comfort of their own homes. Our mission spans coast to coast, supporting patients across the U.S. We are looking for a Health Coach Specialist I for our team in New Haven, Connecticut. This role requires fieldwork 5 days per week at our New Haven, CT or Eastern Corridor (New London/Norwich area) CT offices. Structured family caregiving is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs, while preserving the dignity, self-respect, and privacy of the patient. The Health Coach Specialist will be a vital part of our operations in Connecticut. Responsibilities include performing orientations, providing an assessment of new patients to our program, and training our caregivers to ensure they provide the best service possible to their loved ones.

Requirements

  • 2+ years of experience working with frail or elderly disabled adults and caregivers
  • Abilty to travel thorughout the New Haven area and surrounding cities
  • Valid driver’s license with an insured vehicle for travel within the state
  • Excellent verbal and written communication, organizational and time management skills
  • Strong critical thinking and problem-solving skills
  • Proficient in Microsoft (i.e., Word, Excel, Outlook)
  • Excellent training skills
  • Team-player mentality

Nice To Haves

  • Previous field experience in a medical or case management environment
  • Experience with data input, case review/case note entry; Salesforce experience is a plus
  • Hands-on experience in home care
  • Working knowledge of ADL/ IADL assessments

Responsibilities

  • Home orientations involving signing of paperwork
  • Initial assessments during orientations
  • Periodic supervisory visits / check in for on-care cases
  • Training caregivers / patients as required per case
  • Interacting with the caregiver and other stakeholders, as needed
  • Engaging in interdisciplinary meetings with the care managers, caregivers, and patients
  • Collaborate in the care practice and communicate impressions of cases to appropriate staff
  • Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
  • Wear multiple hats as operations develop
  • Travel within the state of Connecticut.
