Under the general supervision of the RN and /or Physician the Health Care Coordinator will support providers in the provision of health care including recording vital signs, discussing chief health concerns, performing testing, and assisting with procedures and documentation for designated approved OASAS programs. Assesses, plans, evaluates, and delivers patient care in collaboration with RN and/or Physician. Additionally, he/she is a member of the interdisciplinary clinical team at their assigned facility and participates in case conferences, treatment, and discharge planning meetings. The incumbent will serve as a liaison between the agency’s clinical treatment team and the external primary medical care provider ensuring continuity of care for clients with medical conditions. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Requirements

  • BLS certification
  • High School Diploma or Equivalent
  • At least One (1) year experience working in a human services program in a caregiving role
  • Available to work a flexible schedule as needed in response to participant and staff needs
  • Computer literacy including proficiency in Microsoft Office Suite and EHR
  • Ability to put patients at ease and explain physician’s instructions
  • Ability to multi-task with excellent organizational skills
  • Ability to maintain the confidentiality of patient records
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds
  • Knowledge of medical terminology
  • Team player and ability to work independently
