About The Position
Astoria Senior Living, an Agemark Senior Living community, is hiring a Full-Time Health and Wellness Director to lead our wellness team and ensure residents receive exceptional, person-centered care. Requirements
- Active RN or LPN license valid in California.
- At least 1 year of experience working with seniors and a strong understanding of their health and wellness needs.
- At least 2 years of leadership or management experience.
- Strong knowledge of medications, dosages, potential side effects, and safe medication practices.
- Excellent communication, organization, problem-solving, and leadership skills.
- Compassion, Accountability, and Personal Investment in providing excellent resident care.
- Comfort using technology and learning new systems.
Nice To Haves
- Flexibility to work evenings or weekends and serve as Manager on Duty as needed.
Responsibilities
- Lead, mentor, train, and support wellness team members while building a culture of Excellence and Team Spirit.
- Develop and maintain individualized service plans based on resident needs, assessments, and physician orders.
- Serve as a trusted partner for residents, families, team members, physicians, and healthcare providers.
- Ensure resident records, service plans, documentation, and nursing services meet state regulations and Agemark standards.
- Evaluate prospective and current residents to ensure we can safely and successfully meet their care needs.
- Promote safe and accurate medication management while monitoring for potential concerns.
- Provide hands-on support when needed, respond to resident emergencies, and ensure appropriate follow-up and documentation.
- Identify potential risks and lead proactive programs that support residents’ health, independence, safety, and quality of life.
