About The Position

CountryHouse Lincoln III – 70th & O is seeking an experienced RN or LPN licensed in Nebraska to join their team as a Health and Wellness Director. The Health and Wellness Director will lead the Wellness Team, oversee resident care, build strong relationships with residents and families, conduct assessments, manage medications, maintain compliance with state regulations, provide hands-on support, and promote resident wellness through various programs.

Requirements

• Active Nebraska RN or LPN license.
• At least one year of experience working with seniors.
• 2+ years of management or team leadership experience.
• A strong understanding of medications, dosages, and potential side effects.
• Excellent communication, organization, problem-solving, and technology skills.
• A commitment to dignity, independence, and person-centered care.
• Availability to work evenings or weekends and serve as Manager on Duty when needed.

Responsibilities

• Lead the Wellness Team: Train, mentor, and support team members while promoting accountability, growth, and teamwork.
• Oversee Resident Care: Develop and maintain personalized service plans based on resident needs and physician orders.
• Build Strong Relationships: Communicate regularly with residents, families, team members, and healthcare providers.
• Conduct Assessments: Evaluate prospective and current residents to ensure their care needs can be safely met.
• Manage Medications: Oversee medication administration practices and ensure accuracy and compliance.
• Maintain Compliance: Keep documentation and resident records accurate and aligned with state regulations.
• Provide Hands-On Support: Assist with resident care and respond to emergencies when needed.
• Promote Resident Wellness: Support programs that encourage health, safety, independence, and quality of life.
