About The Position

CountryHouse Residence in Elkhorn, NE is seeking an experienced Health & Wellness Director to lead their wellness team and ensure residents receive compassionate, person-centered care. Requirements

Current RN or LPN license in Nebraska required.
At least one year of experience working with seniors and a strong understanding of their health and wellness needs.
At least two years of experience leading, supervising, or developing a team.
Strong understanding of medications, resident assessments, care planning, and clinical documentation.
Excellent communication, organization, problem-solving, and relationship-building abilities.
A commitment to resident dignity, independence, and living the EPIC ACTS values.
Ability to work evenings or weekends and serve as Manager on Duty as needed.

Responsibilities

Lead, mentor, train, and support the wellness team, promoting high standards of resident care.
Develop and maintain individualized service plans based on resident needs, assessments, and physician orders.
Communicate with residents, families, team members, physicians, and healthcare providers to coordinate care.
Ensure documentation, resident records, medication management, and wellness practices meet state requirements and company standards.
Complete assessments for current and prospective residents and identify changes in care needs.
Oversee safe and accurate medication practices and provide education and guidance to the team.
Identify residents at risk, develop proactive care plans, and respond appropriately to emergencies.
Remain visible and engaged throughout the community, providing direct support when needed and ensuring quality care.
