Requirements

  • AA degree in related field
  • 3 years of relevant management, progressive supervisory experience, and staff development experience.
  • Ability to create and deliver curriculum and staff development material.
  • Two years of experience working with marginalized populations.
  • Knowledge of electronic records management
  • Ability to de-escalate situations.
  • Ability to follow confidentiality rules and agency policies and procedures.
  • Willingness to work with residents who have both substance use and mental health disorders; ability to show compassion and professionalism with residents who may be angry, depressed, distraught, frustrated, suicidal, and/or under the influence of drugs and/or alcohol.
  • Willingness to work with individuals who have criminal histories and/or identified criminogenic risk factors.
  • High level of written/verbal communication, customer services skills, efficiency with independent task management and ability to show attention to detail.
  • Demonstrated ability to provide positive, enthusiastic, and motivational counseling and evidence-based practice intervention.
  • Duties require the use of standard office furniture and equipment (i.e., desk, filing cabinet, laptop computer, printer, telephone, cellular phone, fax machine, copy machine, etc.)
  • Proficiency in Microsoft Office Suite, Outlook, and Internet browser navigation
  • Maintain a high level of collaboration and communication with providers in the community.
  • Utilize effective management strategies to ensure unit efficiency and success.

Responsibilities

  • Oversee the coordination of services, including internal and community-based medical and behavioral healthcare, transportation, records requests and storage, education and employment services, case management, discharge planning and aftercare services and referrals.
  • Support program development, operation, and quality assurance by tracking/measuring outcomes, reviewing policies, and ensuring compliance with contracting entities and all governing regulations.
  • Supervise employees.
  • Ensure coordination of all resident physical/behavioral healthcare services on your unit, including telehealth, clinic, outside/visiting provider appointments, transportation, and educational services.
  • Ensure high level of cross-departmental communication and collaboration for continuity of care.
  • Ensure proper policy and procedures, forms and other materials needed to consistently perform the functions of the unit are in place, up-to-date, and reviewed bi-annually for relevance.
  • Ensure all staff are trained in current policy and procedure and can perform to expectations.
  • Add, evaluate, or change services as needed to meet the changing needs of the population and organization.
  • Maintain quality of care consistent with or exceeding standards set by Washington State.
  • Collaborate with community partners to ensure mutual coordination of care needs are addressed.
  • Implement and/or approve program policies and procedures as directed and consult on the development and improvement of policies and procedures that maximize the safety and welfare of the residents, staff, and visitors to the program.
  • Ensure compliance with all regulatory guidelines.
  • Implement necessary quality and inventory control procedures to ensure needs are met, including adequate medication/supplies at all times, error/incident tracking abilities.
  • Ensure the unit is properly staffed and responsible fiscal management is practiced.
  • Serve on an on-call basis, 24 hours a day, to work any shift in the event of scheduling difficulty, crisis, or work stoppage.
  • Oversee and manage personnel processes pertaining to the selection, orientation, training, supervision, and ongoing evaluation of clinical and education program staff.
  • Review performance measurement data with the director and continually work to collect, analyze, and disseminate program data to improve service quality.
  • Participate on the program leadership team and oversee the coordination of unit functions.
  • Coordinate closely and collaboratively with representatives of the support functions of PHS including Mission Services, Behavioral Health, Information Technology, Human Resources, and Communications on an as needed basis or per the request of the program director.
  • Attend PHS meetings as appropriate or upon request and participate in work groups and committees organized by senior leadership and other external agencies as appropriate.
  • Attend stakeholder and partner meetings and events as needed.
  • Deliver presentations as needed to enhance program reputation and further integrate the program within the community-at-large.
  • Maintain accurate, timely, and complete documentation of service.
  • Ensure all necessary notifications are completed for the unit.
  • Ensure all information is accurate and updated from intake to discharge for continuity of care.
  • Record routine case notes and touch points documenting progress and services.
  • Review reports with team to identify service gaps and necessary support services.
