About The Position

The RN Care Coordination Resource Nurse provides districtwide nursing expertise, care coordination, case management, and collaborative nurse support in the school environment. The Resource Nurse builds trusting relationships with students and families and helps identify and address barriers that may affect a student’s ability to learn and participate fully in school. This role is grounded in collaboration, compassion, and a student-centered approach. The RN Care Coordination Resource Nurse brings clinical knowledge, nursing judgment, and strong communication skills to help students and families feel supported and connected. This position includes training, orientation, and a supportive work environment with an established nursing team. This position offers a competitive salary and benefits.

Requirements

  • Registered Nurse (RN) required
  • Bachelor’s Degree in Nursing
  • Ability to read, interpret, and apply student performance data to instructional planning and execution
  • Strong written and verbal communication skills; excellent teamwork skills
  • Driver’s license and own transportation required
  • Valid license to practice as a Registered Nurse in Massachusetts
  • Massachusetts Department of Elementary and Secondary Education School Nurse (Levels: All)
  • Current CPR/AED

Nice To Haves

  • Experience as a Registered Nurse in child health, community health, or other relevant clinical nursing setting preferred

Responsibilities

  • Works under the general supervision of the Superintendent, Chief of Pupil Services, and Director of Health, Wellness, and Nursing.
  • Works collaboratively and cooperatively with School Nurses and Building Principals
  • Care coordination and case management, including care planning.
  • Works with a supportive team (orientation and training provided).
  • Supports CSHS grant initiatives.
  • Attends student meetings
  • Collaborates with implementing state-mandated screening programs.
  • Provides case management and care coordination support.
  • Provides support and health education to students and families
  • Supports students returning to school after extended absences
  • Maintains up-to-date school records.
  • Supports or covers the health office when the school nurse is out of office, engaged in activities such as, but not limited to, care meetings, screenings, health teaching, PD, and other grant-related activities.
  • Provides medically prescribed interventions, including medication administration, and care for ill or injured children.
  • Provides emergency health care and referral to staff.
  • Follows the medication administration plan in the school.
  • Acts as a resource in health education to school personnel, students, and families.
  • Uses community resources for referral of students with unmet health needs, including the need for a primary care provider.
  • Serves as liaison between school personnel, family, and community healthcare providers to ensure a healthy school environment.
  • School-based data collection, review, and reporting
