About The Position

The Health Access Aging Services Coordinator plays a key role in supporting older adults living with HIV by facilitating health insurance access and related healthcare services. This position maintains an active caseload of 100+ clients over the age of 50 and provides one-on-one assistance with Medicare, Medicaid, private insurance enrollment, and Social Security Disability Insurance (SSDI) navigation. The Coordinator works closely with CHN departments, community partners, and state agencies to ensure smooth client transitions and warm handoffs. This role is ideal for a professional with a strong understanding of aging-related issues, HIV/AIDS, Medicare, and public health systems. The successful candidate will be organized, compassionate, and adept at helping clients overcome complex barriers to care. Performance in this position is measured through formal supervision, electronic data analysis, and routine quality assurance auditing.
