About The Position

The HCBS Coordinator supports the Program Manager in coordinating Home and Community-Based Services (HCBS) operations, ensuring quality care delivery to members and effective support of Direct Care Workers (DCWs). This role manages an assigned caseload, oversees day-to-day workforce operations for DCW staff, and coordinates member services, including scheduling, onboarding, compliance, and documentation. The HCBS Coordinator serves as a liaison between families, staff and external partners to ensure continuity of care, service quality, and operational efficiency.

Requirements

  • Minimum High School diploma or GED equivalent and/or Associates degree in a Social Service or related field with working with individuals who are developmentally or physically disabled.
  • Must have 1 – 2 years of verifiable supervisory experience.
  • Must have at least one year of verifiable experience working in the DDD community or more than one year as a direct care worker providing services to individuals who are developmentally or physically disabled.
  • Current knowledge in Arizona DDD regulations and procedures is mandatory.
  • Must be able to function independently have time management skills with the ability to work a flexible schedule which may include nights and weekends.
  • The ability to work effectively with members, providers, office staff, agencies and a diverse population.
  • Must possess the ability to make independent decisions when circumstances warrant such action.

  • Must meet agency licensure requirements for AZ level 1 fingerprinting and direct care professional requirement.
  • Must be proficient using Outlook applications, Microsoft Word, Excel, Power point, and Outlook, including virtual meeting platforms.
  • Must maintain clean driving record and auto insurance and have daily access to vehicle.
  • Must be proficient in program systems and UCP time reporting software.

Responsibilities

  • Supervise and support Direct Care Workers (DCWs).
  • Oversee day-to-day workforce operations, including DCW activity, performance, and accountability.
  • Coordinate and attend onboarding of DCWs in partnership with HR, including Meet and Greets.
  • In conjunction with program leadership participate in communication of terminating DCW’s and offboarding requirements.
  • In conjunction with Adult Program Support Specialist, ensure staff are trained and compliant with DDD, agency, and departmental requirements (e.g., Article 9, PROUD, CPR/FA, DCW Fundamentals).
  • Manage DCW scheduling to ensure full coverage and prevent gaps in service delivery.
  • Provide direct coverage as needed to maintain continuity of care.
  • Monitor and ensure accuracy of payroll and timesheet reporting, including required documentation and approvals.
  • Maintain tracking and reporting of DCW active/inactive status and workforce data.
  • Support billing processes and authorization tracking to ensure accurate and timely service delivery.
  • Serve as Spoke Choice system administrator, ensuring system accuracy, compliance, and staff support.
  • Ensure accurate account of authorization hours and billing requirements.
  • Ensure compliance with EVV requirements and proper system documentation.
  • Support payroll processing by ensuring timesheets and related documentation are complete and submitted within timelines.
  • Support and report employee relations matters, leave request, work injury, etc. in coordination with program leadership and HR.
  • Maintain compliance with contractual, legal, and regulatory requirements.
  • Submit facility, vehicle, and IT maintenance requests as needed.
  • Maintain reliable attendance and dependability.
  • Ensure services align with UCP’s mission, vision, and values.
  • Serve as liaison between members, families, DCWs, and external partners to ensure continuity of care through relationship management.
  • Manage an assigned caseload, ensuring services are delivered in alignment with member needs and support plans.
  • Oversee member intake processes and ensure completion of intake and outtake documentation.
  • Prepare for and participate in PCSP/ISP meetings, including development of outcomes and teaching strategies.
  • Maintain ownership of PCSP processes, ensuring timely coordination, documentation accuracy, and submission to DDD and families.
  • Coordinate with DDD support coordinators and families regarding service planning and scheduling.
  • Monitor implementation and documentation of member outcomes in alignment with service plans.
  • Conduct QA monitoring in accordance with DDD requirements, including ATC monitoring, staff supervision, and home visits.
  • In conjunction with Support Specialist, ensure member files (electronic and hard copy) are accurate, complete, and compliant with agency and regulatory standards.
  • Monitor medication administration and documentation to ensure compliance with legal and medical requirements.
  • Ensure accurate and timely recordkeeping across all program documentation and reporting systems.
  • Respond immediately to health and safety concerns; document and report incidents in accordance with policy and timelines.
  • Report suspected abuse or neglect in accordance with UCP policy, DDD requirements, and state law.
  • Communicate professionally with all stakeholders, including members, families, staff, and external partners.
  • Participate in strategic planning and process improvement initiatives.
  • Actively engage in program activities and support member participation and outcomes.
  • As requested, may serve as a back-up for the HCBS program leadership and Direct Care Workers.
  • Perform other related duties as assigned.
