About The Position

Our Organization Wee Care offers Therapy Services and Home and Community-Based Services using evidence-based treatment models to help individuals achieve and maintain the highest level of independence. Wee Care has been serving families in our community and surrounding communities since April 2006. Role Summary The HCBS Client Care Coordinator serves as the primary relationship owner for assigned members and providers, helping ensure consistent, high-quality services from intake through ongoing care coordination. Key responsibilities include coordinating intake activities; attending planning meetings and capturing habilitation goals/tasks in real time when possible; documenting communication and service activity; supporting provider orientation and re-orientation; tracking member/provider needs; and following up to support service continuity and retention.

Requirements

  • Direct Support Provider experience
  • Background in care coordination, case management, intake, customer service, healthcare support, human services, or related work
  • Compassionate, organized, responsive; strong follow-through and attention to detail
  • Clear written/verbal communication; comfortable working independently and with a team
