The HCBS Client Care Coordinator serves as the primary relationship owner for assigned members and providers, helping ensure consistent, high-quality services from intake through ongoing care coordination. Key responsibilities include coordinating intake activities; attending planning meetings and capturing habilitation goals/tasks in real time when possible; documenting communication and service activity; supporting provider orientation and re-orientation; tracking member/provider needs; and following up to support service continuity and retention.
