The Case Manager is responsible for teaching and enhancing life skills through regular communication and face-to-face interactions with the client. This role involves providing recovery coaching, life coaching, and support to clients, assisting them in developing goals related to various aspects of life, and supporting them towards these goals. The Case Manager will provide face-to-face support, be available for phone or text support and crisis management, and intervene in crisis situations. They will also send weekly updates to the client’s treatment team, assist clients in organizing their day and developing healthy routines, track appointments, and support medication compliance. Additionally, they will encourage engagement in activities of daily living, serve as liaisons, accompany clients to recovery-related activities, attend group supervision, track billable hours, and provide detailed documentation. The role requires listening to clients, building trust, identifying areas for growth, providing feedback on recovery progress, offering stage-appropriate recovery education, and facilitating the transition to client-developed recovery plans. The Case Manager will also provide information about housing, employment, and social services, and assist individuals resistant to abstinence due to obligations. They will help develop recovery support resources, enhance cooperative relationships, cultivate volunteer opportunities, and prepare clients for discharge. Participation in staff development and staying knowledgeable about current information in mental illness and substance abuse is expected. The role utilizes motivational interviewing and the Stages of Change model to assist clients with various issues. Protecting client privacy in accordance with company policies and federal/state law is paramount.
