About The Position

The Case Manager is responsible for teaching and enhancing life skills through regular communication and face-to-face interactions with the client. This role involves providing recovery coaching, life coaching, and support to clients, assisting them in developing goals related to various aspects of life, and supporting them towards these goals. The Case Manager will provide face-to-face support, be available for phone or text support and crisis management, and intervene in crisis situations. They will also send weekly updates to the client’s treatment team, assist clients in organizing their day and developing healthy routines, track appointments, and support medication compliance. Additionally, they will encourage engagement in activities of daily living, serve as liaisons, accompany clients to recovery-related activities, attend group supervision, track billable hours, and provide detailed documentation. The role requires listening to clients, building trust, identifying areas for growth, providing feedback on recovery progress, offering stage-appropriate recovery education, and facilitating the transition to client-developed recovery plans. The Case Manager will also provide information about housing, employment, and social services, and assist individuals resistant to abstinence due to obligations. They will help develop recovery support resources, enhance cooperative relationships, cultivate volunteer opportunities, and prepare clients for discharge. Participation in staff development and staying knowledgeable about current information in mental illness and substance abuse is expected. The role utilizes motivational interviewing and the Stages of Change model to assist clients with various issues. Protecting client privacy in accordance with company policies and federal/state law is paramount.

Requirements

  • Degree from an accredited college/university and/or High School GED.
  • Minimum of 2 years of experience in chemical dependency and/or direct care settings.
  • Knowledge of the Principles and Practices of the discipline.
  • Knowledge of Joint Commission Standards.
  • Proficiency in Communication & Written skills.
  • Knowledge of State & Federal statutes regarding patient confidentiality laws.
  • Knowledge of addictions and mental health complications.
  • Knowledge of community health resources that support life improvement in physical, emotional, mental and spiritual health, economic security, employment, positive social and family interdependence and positive cultural identity including: Knowledge of referral procedures and admittance criteria for health care and community providers.
  • Ability to network and collaborate with healthcare and other social and cultural agencies that can provide services to the patients and families.
  • Ability to determine if a medical emergency or mental health crisis exists and to take appropriate action, when necessary.

Responsibilities

  • Providing recovery coaching, life coaching, and support to clients.
  • Assisting in developing goals with the client related to various aspects of life.
  • Supporting the client towards goals, including but not limited to; spiritual health, mental health, vocational/educational/volunteer work, social support, hobbies and leisure activities, recovery goals, physical health, and any other specific areas that apply to the individual client.
  • Providing face-to-face support for a predetermined number of hours per week to the client.
  • Being available to speak to the client by phone or text for support and crisis management.
  • Intervening in crisis situations and engaging in conflict resolutions to assist their client.
  • Sending weekly updates to the client’s treatment team.
  • Assisting the client in organizing and structuring their day, developing healthy routines and habits.
  • Keeping track of clients’ doctor and therapy appointments, ensuring they are attending, and providing transportation if needed.
  • Supporting clients in medication compliance with regular check-ins as needed, reminding clients when their medication refills are due and assisting them in picking up medications.
  • Encouraging clients in activities of daily living including but not limited to self-care, grocery shopping, and budgeting.
  • Serving as liaisons to other individuals in the client’s life.
  • Encouraging engagement in pro-recovery behaviors and accompanying the client to recovery-related activities.
  • Attending weekly group supervision, as required, with Clinical Supervisor during one of the two group supervision times that are offered.
  • Keeping track of all the hours that are spent with the client and any other billable services and submitting the hours for payroll.
  • Providing specific, detailed documentation describing the activities/services provided for the client.
  • Seeking consultation when needed and deferring to the direct supervisor prior to suggesting referrals to clients.
  • Listening to the client, being trusted with confidence and able to identify areas of potential growth.
  • Providing feedback on the client’s recovery progress, identifying areas which have presented or may present roadblocks to continued success.
  • Providing stage-appropriate recovery education.
  • Facilitating the transition from a professionally directed treatment plan to a client-developed and directed personal recovery plan as needed.
  • Providing knowledgeable information, for individuals and/or for their families, about sources of safe housing, recovery conducive employment, health and social services, and recovery support.
  • Providing service for those resistant to remaining abstinent from drugs and/or alcohol, but who must do so due to legal, medical, family or contractual obligations.
  • Helping develop and expand recovery support resources, enhance cooperative relationships between professional service organizations and local recovery support groups, cultivate opportunities for people in recovery to participate in volunteerism, and performing other acts of service to the community.
  • Preparing clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and post discharge requirements; orienting and training family members; providing resources.
  • Participating in staff development and being responsible for knowledge concerning current information in mental illness and substance abuse and treatment and related behavioral problems specific to the age and population served.
  • Engaging with clients utilizing techniques of motivational interviewing to empower clients while using the ‘Stages of Change’ model to understand and to assist them with issues related to housing, benefits, employment, life skills, treatment for substance abuse and mental or physical health issues, etc.
  • Protecting the privacy of all client information in accordance with GRNH, LLC’s privacy policies, procedures, and practices, as required by federal and state law, and in accordance with general principle of professionalism as a healthcare provider.
