About The Position

This is a highly responsible professional position in the Bureau of Communicable Diseases, HIV/AIDS Section, strengthening linkage services for HIVE-infected persons in accordance with the HIV National Strategic Plan, Goal 2: Improve HIV-Related Health Outcomes of People with HIV (PWH). This position will oversee direct linkage to care activities throughout the state. This position reports to the Linkage Team Leader for the Prevention Section. Monitors and examine the system of linkage to care in each county, which includes identifying barriers to linkage, adherence, and retention gaps in services; and trends in demographics, geography, origin of test sites or medical provider. Provides training and technical assistance to regional and local linkage field staff on linkage, adherence, and retention expectations and outcomes. Monitors linkage to care in linkage modules (CTLS) and assist areas of the state that have lower than average linkage and retention rates by providing technical assistance/training. Conduct quarterly conference calls with linkage to care field staff to share program updates and best practices. Collaborates with the Surveillance Section on Data to Care (02C} linkage activities and outcomes. Monitors and analyzes linkage, D2C, and re-engagement to care data related to Ending the Epidemic (EHE). Generates weekly and monthly reports identifying those in need of medical care and relays information to field staff to facilitate linkage to those services. Monitors and evaluates the outcomes of area re-engagement in care pilot projects. Participates in community/state meetings, workgroups, and/or calls with designated DOH HIV/AIDS staff, national partners, and community members that address linkage to care/02C, retention, and adherence for high-risk populations. Assist with the development of D2C materials and resources for linkage and re-engagement in care. Identifies strategies for successful linkage and re-engagement in care to improve continuity of care for PWH. Provides technical assistance to linkage field staff on D2C linkage policies and procedures. Reviews linkage error reports and ensures D2C records are followed up on and dispositioned according to Department standards. Collaborate with STD and Hepatitis Section to review weekly STARS Critical Task List. Prompts linkage staff to follow-up on open records and disposition them in a timely manner. Provides updates Department's Standards of Care and Linkage Module Resource Guide. Attends meetings with Data Integration Team to review enhancements to the linkage reporting system. Updates and maintains linkage field staff contact listing. Assist in the preparation of quarterly/annual reports and other written documents as assigned. Ensure updated Information on the linkage team is maintained on the website. Perform other related duties as assigned.
