About The Position

This is an ongoing coverage need for a Gerontology Nurse Practitioner (NP) or Physician Assistant (PA) within a PACE Program. The role involves outpatient coverage with some off-site visits to Skilled Nursing Facilities (SNF) and Assisted Living facilities. The average patient load is 4-6 patients per day. The position is open due to a long-term permanent search. Expedited/Temporary privileges are available.

Requirements

  • Active Oregon licensure
  • Board certification
  • AHA BLS certification
  • Minimum 1-year Geriatric experience or equivalent
  • Active and unencunbered advanced practice registered nurse (APRN) license in Oregon
  • National certification as an NP in a relevant specialty area by a recognized certifying body (e.g., American Academy of Nurse Practitioners [AANP])
  • Active and unencumbered physician assistant (PA) license in Oregon
  • National certification from the National Commission on Certification of Physician Assistants (NCCPA)

Nice To Haves

  • FT availability preferred
  • 3-4 days per week will be considered
  • Minimum 2 weeks per month availability

Responsibilities

  • Conduct comprehensive patient assessments to diagnose and treat a variety of medical conditions
  • Develop and implement individualized treatment plans
  • Order and interpret diagnostic tests
  • Prescribe medications and manage chronic conditions
  • Provide patient education and counseling on healthy lifestyle choices
  • Perform procedures as permitted by state regulations and APP specialty
  • Collaborate with physicians and other healthcare providers to ensure coordinated care
  • Document patient care accurately and thoroughly
