About The Position

Under general direction, this role provides short-term case management services to GAIL clients, assists with community support groups, and offers information and referral services to Greenbelt seniors, disabled adults, and their caregivers.

Requirements

  • Bachelor’s Degree from an accredited college or university in Social Work, Public Health, or other Social Science Field with coursework in geriatrics or Bachelor’s Degree in Nursing preferred or State of Maryland Health Worker Certification or Department of Labor Community Outreach Coordinator Certification.
  • Training, certification or experience in providing case management services.
  • Training, certification in community nursing.
  • One to three years of responsible experience.
  • Any combination of education, training and experience which provides the required knowledge, skills, and abilities to perform the essential functions of the job.
  • LBSW, Case Management, Service Coordination Certification, Mental Health First Aide, Dementia Friends or Dementia Practitioner Certification or a combination of the above certification.
  • Knowledge of Federal, State, and Local Community Resources.
  • Knowledge of Geriatric/Senior services and resources.
  • Knowledge of Case Management.
  • Knowledge of pertinent Federal and State statutes and City ordinances.
  • Knowledge of all computer applications and hardware related to performance of the essential functions of the job.
  • Knowledge of Department organization, standard operating guidelines and policies, rules, and regulations.
  • Skill in preparing client progress notes, clear and concise reports, correspondence, and other written materials.
  • Skill in communicating orally and in writing with internal staff, clients, the public, and other departmental staff in order to give and receive information in a courteous manner.
  • Skill in effective delivery of case management services in a variety of settings.
  • Knowledge of Federal, State, and County resources or ability to obtain information on needed resources.
  • Skill in operating and maintaining all assigned equipment required to perform the essential functions of the job.
  • Ability to provide case management services with diverse clients.
  • Ability to be tactful, courteous, and poised in difficult situations.
  • Ability to establish and maintain effective working relationships with a variety of people.
  • Frequently required to stand; walk; sit; use hands to finger, handle, feel or keyboard; speak and hear; lift and/or move up to 25 pounds.

Responsibilities

  • Provides initial intake and assesses benefits analysis and appropriate program linkage.
  • Provides information and referral services to Greenbelt seniors, disabled adults, and caregivers.
  • Provides short-term case management services to Greenbelt seniors, disabled adults, and caregivers.
  • Completes on-site client visits as necessary.
  • Conducts cognitive and mental health screenings as necessary.
  • Completes clear and concise client progress notes.
  • Assists in the implementation of community health and wellness programming.
  • Co-facilitates support groups, teaches educational workshops, and hosts webinars.
  • Works with GAIL Community Case Manager to provide case management services to seniors, disabled adults, and caregivers.
  • Maintains a current list of Federal, State, and County resources for use with seniors and caregivers.
  • Performs other related duties as assigned.
