About The Position

The Manager, Foundation Partnership and Community Outreach serves as a strategic connector between Compassus and nonprofit foundations associated with its joint venture (JV) partners and local markets. This role does not directly operate those foundations but instead ensures their activities and resources are aligned with Compassus’ mission, compliance standards, and community priorities. Working across Compassus markets, JV partners, and community organizations, the manager provides training, guidance, and oversight on how foundation resources can be accessed and utilized effectively. In addition, this role leads outreach and education initiatives that foster community understanding of hospice and home-based care, bringing to life the Care for Who I Am philosophy.

Requirements

  • Bachelor’s degree in nonprofit management, healthcare administration, public health, communications, or related field required.
  • 5+ years of experience in foundation relations, community outreach, grant management, or healthcare philanthropy.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
  • Strong written and verbal communications.
  • Ability to understand, read, write, and speak English.
  • Articulates and embraces integrated healthcare at home philosophy.
  • Strong knowledge of compliance and regulatory standards in the philanthropic and healthcare sectors.
  • Proven ability to influence and align diverse stakeholders, including donors, JV partners, and local market leaders.
  • Skilled communicator with strong project management and reporting capabilities.
  • Proficiency with CRM systems, donor databases, Microsoft Office Suite, and fundraising platforms.
  • Strong alignment with the Compassus mission and Care for Who I Am culture.
  • Demonstrated ability to balance compassion, compliance, and operational excellence.
  • Experience in developing impact reports and managing partner or donor relationships.
  • Passion for strengthening community understanding and access to home-based care.
  • Ability to understand, read, write, and speak English.

Responsibilities

  • Serve as a liaison between Compassus and associated JV/nonprofit foundations.
  • Guide partners and local teams on how to request and use foundation resources compliantly and effectively.
  • Develop and implement partnership strategies that ensure foundation activities align with Compassus’ mission and community needs.
  • Provide training and tools to local leaders, empowering them to maximize the impact of foundation support.
  • Develop, vet, present and deliver reports and presentations on the success Compassus’ alignment with various foundations.
  • Identify, coordinate, and support community outreach initiatives that elevate awareness of hospice and home-based care.
  • Lead annual reporting on the collective impact of foundation partnerships and outreach efforts.
  • Identify, coordinate, and support community outreach initiatives that elevate awareness of hospice and home-based care.
  • Partner with marketing, clinical, and local teams to implement education programs, sponsorships, and events that extend community impact.
  • Track, analyze, and report outcomes of outreach initiatives to demonstrate alignment with Compassus’ mission and values.
  • At a corporate level, monitor how various foundation funds are allocated and utilized across JV partners and markets.
