About The Position

The ACT Forensic Registered Nurse position is part of the Assertive Community Treatment (ACT) team, which provides services to individuals with severe and persistent mental illness who have complex physical, psychiatric, and legal conditions. This role requires a high level of medical necessity and involves conducting psychiatric and physical health assessments, coordinating referrals to community physicians, managing and administering medications, and providing a range of treatment, rehabilitation, and support services. The nurse will work within the Community Support Program model, developing strength-oriented treatment plans that address consumer health care needs through community resources. The incumbent is responsible for ensuring that healthcare services align with the mission, goals, and philosophy of the ACT team, providing age and culturally appropriate care, and promoting consumer independence in daily living, interpersonal skills, leisure activities, and physical/mental health maintenance. All contacts made by the incumbent are billable services.

Requirements

  • Registered Nurses Degree
  • At least two (2) years of experience in the mental health field
  • Pennsylvania Licensed in good standing

Responsibilities

  • Coordinates the health care needs of assigned consumers by assessing, planning, implementing, and evaluating psychiatric and medical needs.
  • Maintains records of medical history and ongoing medical records.
  • Completes Nursing Health Assessments for CTT consumers annually and updates as needed, including nutritional assessments and education.
  • Develops and implements individualized health treatment plans.
  • Provides comprehensive service coordination (Case Management) to assigned consumers.
  • Consults with the Team regarding the development and implications of Nursing and Service Plans.
  • Develops comprehensive treatment plans utilizing behaviorally specific language, specific interventions, and measurable goals.
  • Negotiates health care responsibilities with provider agencies and CTT.
  • Facilitates the use of generic services.
  • Participates on assigned teams, providing expertise in physical health.
  • Attends daily staff organizational meetings and treatment planning review meetings.
  • Coordinates consumers' routine medical, vision, lab, and dental appointments, including follow-up protocols.
  • Accompany consumers to appointments when deemed necessary by the nurse and team.
  • Attends psychiatric team meetings to review medication, Personal Plans, and response to treatment.
  • Attends team meetings to review health care needs, consumer response to treatment, and coordinate appointment scheduling.
  • Takes a lead role and/or participates in providing medications, monitoring and administering consumer medications, and coordinating/administering injections.
  • Participates in discharge planning for consumers from acute or extended care facilities.
  • Monitors and provides medical treatment in the community as prescribed by a recognized medical service vendor.
  • Completes appropriate documentation, including developing, reviewing, and revising forms, procedures, and standards of care.
  • Documents services provided according to CTT policy and completes Service Documentation for all billable activities.
  • Serves as a member of the CTT Quality Assessment program.
  • Maintains and updates nurses' section(s) of consumer Personal Records.
  • Completes medication administration records (MAR) and injection (IM) records.
  • Monitors the status of consumers in acute hospital settings.
  • Provides consultation services to community agencies and other teams upon request and as determined necessary.
  • Conducts educational and training sessions with consumers, families, provider agencies, and CTT Staff.
  • Utilizes effective communication skills with consumers, family members, and team members.
  • Performs other duties as required.
