About The Position

Unison Health is seeking a registered nurse to provide comprehensive care to clients in community-based settings. The role serves individuals with mental health and substance use needs and includes physical and mental health assessments, medication administration and education, direct nursing care, and ongoing health monitoring. The nurse will help promote clients’ health and recovery through compassionate, organized care.

The position also maintains the Woodruff Outpatient Medication Room and coordinates care with providers, clients, families, and community resources. The nurse collaborates with clients and the interdisciplinary treatment team on nursing care plans and supports continuity of care and treatment goals.

Requirements

• Associate degree in nursing.
• Current RN license from the State of Ohio.
• Must demonstrate comfort interacting with individuals with mental illness.
• Must establish and maintain effective working relationships.
• Must have excellent customer service skills.

Nice To Haves

• Bachelor’s degree in nursing.
• Previous psychiatric experience.
• Experience working with SMI.
• Certification as a psychiatric nurse.

Responsibilities

• Maintain the Woodruff Outpatient Medication Room, including coordinating medication orders with the pharmacy, managing sample medications, and monitoring inventory and expiration dates.
• Provide nursing services to outpatient and community-based clients, including medication administration, medication education, and ongoing health monitoring.
• Conduct behavioral and physical health assessments.
• Provide crisis intervention and develop nursing care plans in collaboration with clients and the interdisciplinary treatment team.
• Coordinate care and serve as a liaison between providers, clients, families, and community resources to ensure continuity of care and support treatment goals.
• Provide emergency first aid as needed.
• Maintain compliance with agency, state, and regulatory requirements related to nursing services.
