About The Position

Unison Health is seeking a registered nurse to provide comprehensive care to clients in community-based settings. The role includes physical and mental health assessments, medication administration and education, and direct nursing care. The ideal candidate is compassionate, organized, and committed to promoting the health and recovery of individuals with mental health and substance use needs.

The nurse provides outpatient and community-based nursing services and helps maintain the Woodruff Outpatient Medication Room. The role also includes crisis intervention, care planning with clients and the interdisciplinary treatment team, coordination with providers and community resources, emergency first aid, and compliance with applicable nursing regulations.

Requirements

• Associate degree in nursing.
• Current RN license from the State of Ohio.
• Experience working with SMI.
• Comfort interacting with individuals with mental illness.
• Ability to establish and maintain effective working relationships.
• Excellent customer service skills.

Nice To Haves

• Bachelor’s degree in nursing.
• Previous psychiatric experience.
• Certification as a psychiatric nurse.

Responsibilities

• Work in community-based settings.
• Maintain the Woodruff Outpatient Medication Room, coordinate medication orders with the pharmacy, manage sample medications, and monitor inventory and expiration dates.
• Provide nursing services to outpatient and community-based clients, including medication administration, medication education, and ongoing health monitoring.
• Conduct behavioral and physical health assessments, provide crisis intervention, and develop nursing care plans with clients and the interdisciplinary treatment team.
• Coordinate care and serve as a liaison among providers, clients, families, and community resources to support continuity of care and treatment goals.
• Provide emergency first aid as needed.
• Comply with agency, state, and regulatory requirements related to nursing services.
