The ACT Forensic Registered Nurse will be a member of the Assertive Community Treatment (ACT) multidisciplinary team, providing services to individuals with severe and persistent mental illness who have complex physical, psychiatric, and legal conditions. This role involves conducting psychiatric and physical health assessments, managing and administering medications in collaboration with a psychiatrist, providing a range of treatment, rehabilitation, and support services, and sharing shift management responsibilities. The nurse will utilize the Community Support Program model to assess health needs, develop strength-oriented treatment plans, and connect consumers with community resources. A key aspect of this role is ensuring the mission, goals, and philosophy of the ACT team are operationalized, particularly concerning healthcare services, and providing age and culturally appropriate care. The incumbent will work with staff to ensure continuity of care and enhance the consumer's independence in daily living, interpersonal skills, social support, leisure activities, and the maintenance of physical and mental health. All contacts made by the incumbent are billable services.
