Forensic Outreach Worker

About The Position

CHD is seeking a Forensic Outreach Worker to provide direct outreach services to people served who live independently in the community and have a history of, or current, forensic involvement. The role supports individuals’ independence, rehabilitation, and recovery through person-centered assistance, advocacy, and supportive relationships. The worker also coordinates with professionals in the criminal justice system and other community agencies to support individuals and promote compliance with legal matters.

Requirements

A bachelor's degree in Social Work or a related field, and/or relevant experience working with individuals with co-occurring mental health and substance use disorders and forensic involvement.
Strong written, verbal, and interpersonal skills.
Empathy for others and a demonstrated commitment to serving people with significant needs.
Ability to work well within a team model and independently.
Computer skills.
A vehicle for work use and a current driver's license.
Ability to complete medication administration training.
Successful completion of a background record screening process.

Responsibilities

Support people served with activities of daily living.
Administer medications as needed.
Advocate for individuals in various community settings.
Provide opportunities to optimize individuals’ independence, rehabilitation, and recovery.
Develop professional and supportive relationships with people served.
Provide informal counseling and guidance with a person-centered focus.
Liaise with professionals in the criminal justice system—including the Department of Mental Health, police department, probation department, attorneys, and house of corrections—to support individuals and promote compliance with legal matters.
