Forensic Outreach Worker

About The Position

CHD is seeking a Forensic Outreach Worker to provide direct outreach services to people living independently in the community who have a history of, or current, forensic involvement. The role supports individuals in daily living, medication administration as needed, community advocacy, and efforts to optimize independence, rehabilitation, and recovery. The worker builds professional, supportive relationships and provides informal counseling and guidance with a person-centered focus. The position also involves liaising with criminal justice and human services professionals to support individuals and promote compliance with legal matters.

Requirements

Bachelor’s degree in Social Work or a related field, and/or relevant experience working with individuals with co-occurring mental health and substance use disorders and forensic involvement.
Strong written, verbal, and interpersonal skills.
Empathy for others and a demonstrated commitment to serving people with significant needs.
Ability to work well within a team model and independently.
Computer skills.
A vehicle for work use and a current driver’s license.
Ability to complete medication administration training.
Must complete and pass a background record screening process.

Responsibilities

Support persons served with activities of daily living.
Administer medications as needed.
Advocate for individuals in various community settings.
Provide opportunities that optimize individuals’ independence, rehabilitation, and recovery.
Develop professional and supportive relationships with persons served.
Provide informal counseling and guidance with a person-centered focus.
Liaise with professionals in the criminal justice system, including the Department of Mental Health, police, probation, attorneys, and house of corrections, to support individuals and promote compliance with legal matters.
